This is a purely scientific investigation; the report will provide facts relevant to the evidence collected and will not make any assumptions as to organizational factors or human influences related to the failure.
- Causal – a physical failure investigation with an analysis of the factors contributing to the event. This approach will ascertain the facts that led to the failure and identify corrective technical actions. The human and organizational causes will not be investigated.
- Full RCA – a physical failure investigation backed up by a full analysis of the human and organizational factors contributing to the failure. This type of investigation offers the most valuable learning, and the highest level of transparency. The effort required by our clients to help us complete full RCA's must not be underestimated.
